Work collaboratively with the cardiologists to provide direct patient care. Conduct independent office visits and prescribe treatment plan. Determine proper level of E & M service. Coordinate with the MD to modify plans of care based on patient condition and/or response to treatment. Document patient interactions in the electronic medical record. Respond to patient telephone calls. Function as a patient advocate by communicating patient needs to applicable health care team members. Provide patient education. Conduct exercise stress testing and pharmacologic stress testing as needed. Provide weekend on-call coverage. Exemplify MHS values: Patients First, Teamwork, Excellence and To Endure.
REQUIREMENTS: Registered Professional Nurse with licensure in the State of Illinois. Advanced Practice Nurse with licensure in the State of Illinois. A minimum of 5 years of clinical expertise caring for the cardiovascular patient population is recommended. ACLS & BLS certifications. Annual treadmill credentialing as applicable.
